3D Scanned
The Trios 4 by 3Shape produces extremely accurate 3D scans of patients’ mouths, replacing traditional impressions. When merged with the 3D volumes created by the CBCT machine, these 3D scans make digital implant and rehabilitation planning possible. They are an excellent visual tool to show the treatment process.
Full Frames
The taking of well-oriented and accurately lit photographs greatly enhances case planning and the digital design process. Most cases involve a photography session to capture the necessary information for case planning. Dr van Niekerk is fanatical about photography and only uses the best equipment to achieve consistent, quality images.
3D Printed
Able to print a host of different biocompatible materials, the 3D printer allows digital scans and designs to be processed in a clean and accurate workflow. Used to print prototypes, models guides and much more, the power of 3D printing in dentistry is unparalleled, offering novel treatment solutions.
Precise Milling
This PM7 5-axis milling machine from Ivoclar® is a behemoth and central to the clinic’s production facility, capable of milling and finishing a host of materials to an industry-leading standard in accuracy. We produce complex implant frameworks previously outsourced to offshore labs, reducing treatment timeframes and costs.

CASE 1
TREATMENT OBJECTIVE
Our aim was to remove all elements of infection and disease and to restore function as quickly as possible. The complete case was digitally planned prior to the day of surgery. Six fully-guided implants were placed during a two-hour morning surgery. An immediate implant-supported prosthesis was delivered at noon that same day.
COMMENTS
We were challenged with a short treatment timeline as the patient was visiting us from the UK and was on a tight traveling schedule. We accomplished this result for the upper jaw in a single day visit. The lower jaw will be completed similarly when our patient returns.
CASE 2
TREATMENT OBJECTIVE
Our aim was to remove all elements of infection and disease and to restore function as quickly as possible. The complete case was digitally planned prior to the day of surgery. Four fully-guided implants were placed during a morning surgery that took less than 2 hours. An immediate implant-supported prosthesis was delivered the next morning.
COMMENTS
This case illustrates how planned angled implants can effectively provide a stable platform thereby eliminating the need for tissue grafting and the associated protracted healing times.
CASE 3
TREATMENT OBJECTIVE
Our patient endured a long history of oral disease and associated infections. Our aim was to remove all elements of infection and disease and to restore function as quickly as possible. Six fully-guided implants were placed and immediately restored with implant-supported prosthesis in both the upper and lower jaws respectively. These were then converted to final zirconia framework prosthesis as healing and tissue remodeling allowed. The complete timeline for a treatment such as this varies from 6-24 months.
COMMENTS
This was a landmark case for our clinic – being the first fully digitally-planned upper and lower rehabilitation. This case demonstrated the power and effectiveness of digital implant-surgery planning and CAD design software. No adjustments were needed at the placement of the final lower prosthesis.
CASE 4
TREATMENT OBJECTIVE
Our aim was to remove all elements of infection and disease and to restore function to an aged dentition as quickly as possible. Five fully-guided implants were placed during surgery that took less than 2 hours. The patient received a full-arch custom designed and milled provisional on the same day.
COMMENTS
This case illustrates how implant rehabilitation can be done in combination with other crowns to deliver agreeable results early in the treatment process. The final individual crowns and implant-supported bridges were delivered in zirconia once healing and tissue-remodeling has completed.
CASE 5
TREATMENT OBJECTIVE
Our aim with this case was to restore the teeth that were severely damaged due to years of bruxism (grinding). There was also a single tooth missing which was replaced with an implant and implant-supported crown. The crown preparations and fully-guided implant-surgery was completed in 2 hours, the custom-designed provisional bridge, milled and placed later that same morning.
COMMENTS
This case illustrates how we are able to accurately restore a dentition severely affected by bruxism (grinding). A smile design software module was used to conceive the new smile position, which can be extremely challenging with such a limited frame of reference. The final prosthesis consisted of individual crowns on all the damaged upper natural teeth and a single implant-supported crown. We are currently in the process of completing the lower arch.
CASE 6
TREATMENT OBJECTIVE
Our aim with this case was to remove all elements of infection and disease and to restore function as quickly as possible. The complete case was digitally planned prior to the day of surgery. Eight fully-guided implants were placed during a morning surgery that took less than 3 hours. Our patient had a restful afternoon and returned to the practice the following morning to receive full upper and lower implant-supported provisional prosthesis.
COMMENTS
This case illustrates how extremely complex cases can be handled accurately and without major inconveniences to the patient, thanks to the incredible power of digital implant-planning and CAD design software. Full Class I function was restored. Our patient loves his milled long-term PMMA provisionals so much that he requested we make him another set when tissue remodeling is complete.
